Intermediate care

Lewisham Intermediate Care provides help, support and encouragement to help you live independently after a hospital stay or if you are finding it difficult to manage.

Members of the team will work with you to find out what you need and what you want to do at home. Your support from us could last for only a few days or it could last for up to six weeks. Health professionals or social workers will refer you to the Intermediate Care service.

Mental Capacity Act

There is a new law which protects the rights of people who find it hard to make a decision, because of dementia or brain injury for example.

If there is a doubt as to whether someone can make a decision, the new law says he or she must have as much help as possible:

  • must be assessed to see if they can make a particular decision
  • can be assessed to see if they can make complicated or just straightforward decisions
  • must be involved in a decision even if unable to make it for themselves.
