Please find below the serious case review involving a service user in July 2006.
A council spokesperson said: "We unreservedly apologised and accept it should never have happened. Lewisham Council undertook a thorough investigation and has already implemented changes to policy to ensure the chances of such an incident happening again are minimised.
"The two members of staff involved were immediately suspended according to Council policy and have since resigned.
